This form is to be used by professionals to refer service users into the North Region Macmillan Welfare Benefits Service. Please do not forward this referral form to service users to self refer.

Upon receipt of your referral, your service user will be contacted within 2 working days of it being received. Our Macmillan Welfare Benefits Service can assist those affected by cancer and can work out what financial help they are entitled to and help them to access it. We provide confidential advice on which benefits they can claim, help them to apply for benefits and for one-off grants to help with energy costs or travel costs to hospital appointments for example. We can also help appeal against decisions on benefit entitlement.

Please send the referral through as soon as it is agreed in order for us to provide a timely response to the service user.
